Why is the staff member I want to create a form for greyed out?

This means that particular staff member has already had a performance management form created for them. Go to ‘Performance Management’ > ‘Active Documents’ and you should see their name in at least one of the tables. To edit the document that has already been started for them click ‘Action’ and then ‘Open’. To create a new review without editing the original document click ‘Action’ and then ‘Create Review’ (note this option will only be available if an original document has already been created).
