This will be down to one of two things:

  1. You have not yet selected a form type.
  2. You have not got any roles linked to the form type you have chosen.

If it’s the latter, you will need to be logged on as an administrator to fix it. Go to ‘Administration’ > ‘Teaching and Learning Admin’ > ‘Form Types’. Locate your form type and click ‘Edit’. Click on the ‘Staff Roles Linked to this form type’ dropdown and select the roles for which you would like this form type to appear. Click ‘Save’.
